When receiving a massage from Relax At Home Massage, you are releasing in full all claims and rights and you acknowledge that massage therapy is not a substitute for medical attention. You agree to release and forever discharge Relax At Home Massage, affiliates and representatives from any and all rights, claims, demands, liabilities, causes of action and damages of any kind whatsoever, known or unknown, existing now or arising in the future, resulting from or related to, any claimed personal injury or damage due to the alleged negligence from massage therapy treatments and services. Before the massage, you will state to the best of your knowledge all know physical conditions that you have and agree to advise Relax At Home Massage and its affiliates of any changes therein.
